Customer Notification: If you are a new customer or a past customer who has not submitted samples since 2021, a completed W-9 (W-8 for foreign entities) is required before we begin to process/analyze samples. If you don’t have a completed form to submit as a PDF file via email, links are included below.

3 Basic Swab Kit includes: 1 foam cooler, 6 swabs, 8 specimen cups for drip samples, 1 cool-pack, sampling/submission information, and items needed to take, prep, and ship the samples.
Shipping Reminders:
Ship samples overnight with cold ice pack. Label outside of box with “water samples.”
